Maintenance Request Form

DISCLAIMER, ACKNOWLEDGEMENT & INDEMNITY By submitting this work order request to the Head Office Rental Support Team, I acknowledge and confirm the following: 1. Owner Approval & Contractor Selection I have obtained approval from the property owner/my Client to proceed with these works on their behalf. My direction to use a specific Contractor is based on my own assessment that they are suitably licensed, qualified, and insured to carry out the works. I acknowledge that @realty Pty Ltd does not verify the credentials, licenses, insurances, or qualifications of the nominated Contractor. 2. Accuracy & Completeness of Information I have provided the Head Office Team with all the required and accurate information necessary to engage the nominated Contractor on behalf of my Client. I understand that @realty Pty Ltd is not responsible for any delays, misinterpretations, or errors arising from incomplete or inaccurate information provided in this form. 3. No Expectation of Additional Instructions or Advice I acknowledge that the Head Office Team are not qualified builders, plumbers, electricians, or professional tradespeople. I understand that all work is being booked solely based on the instructions I provide in this form. I have not requested and do not expect the Head Office Team to provide the Contractor with any additional instructions beyond what is specified in this form. 4. Responsibility for Work Orders & Changes I understand that any work orders related to property management maintenance are issued based solely on my instructions. Any modifications to this request must be provided in writing to the Head Office Team, and I accept responsibility for ensuring such instructions are clear and timely. 5. Payment & Financial Responsibility If the works are being booked in relation to maintenance or repairs requested by the tenant, I acknowledge that initial payment for the works will be deducted from the property owner’s rental income held in trust. Where applicable, I will direct the Head Office Team to issue an invoice to the tenant to recover costs for my Client. @realty Pty Ltd bears no financial responsibility for any disputes or unpaid invoices related to this maintenance request. 6. Indemnity & Release I agree to indemnify and hold harmless @realty Pty Ltd, its directors, officers, employees, and agents from and against any and all claims, demands, losses, damages, liabilities, costs, and expenses (including legal fees) arising out of or in connection with: The selection, engagement, or performance of the Contractor; Any damage, injury, or loss (whether to persons or property) arising from the maintenance work; Any failure by the Contractor to comply with applicable laws, regulations, or industry standards; Any contractual or financial obligations entered into between myself and the Contractor; Any errors, omissions, or miscommunication in the information provided in this form. By submitting this form, I acknowledge that I have read, understood, and agreed to the above terms.
